Regional West Physicians Clinic to Partner with ChartSpan

SCOTTSBLUFF, Neb., ― Regional West Physicians Clinic-Family and Internal Medicine clinics have partnered with ChartSpan, a preventative care service provider, to offer comprehensive care management programs to Medicare patients.
ChartSpan is currently the largest full-service Chronic Care Management (CCM) organization in the U.S., serving 175+ health systems and tens of thousands of Medicare beneficiaries.
Chronic Care Management assists Medicare patients with two or more chronic conditions receive preventative care each month. A monthly copay may be associated with the program, dependent upon your plan, and deductibles do apply. A ChartSpan representative will review cost sharing potential during your call.
The collaboration between ChartSpan and Regional West Physicians Clinic will emphasize:
1. Increased patient engagement: Enrolled patients will have access to a 24/7 phone and text line for after-hours care and will receive monthly calls from a designated care manager. During these calls, they can ask questions, share health updates, and benefit from assistance with appointment scheduling, health goals, and medication management.
2. Personalized care: Patients will receive personalized care plans, based on recommendations from their providers and the patients’ own preferences. These care plans will reflect their medical history, desired outcomes, and values and goals, empowering the patient to take control of their own health.
