Specimens must be clearly labeled.

  • If your facility is using requisitions with unique crack and peel labels, these can be used to label each specimen submitted for testing. NOTE-Specimen(s) must be related to the requisition. Be sure that the label placed on the tube matches the number on the requisition. If specimen being submitted is for blood bank testing (e.g., obstetric panel, type and Rh, antibody screen), tube must be labeled with patient's first and last name, date of birth, date and time of collection, and initials of phlebotomist.
  • If your facility is not using requisitions with crack and peel labels, specimen(s) must be labeled with a minimum of the first and last name. Additional desirable information includes date and time of collection and initials of phlebotomist. Blood Bank specimens must be labeled with patient's first and last name, date of birth, date and time of collection, and initials of phlebotomist.
  • If specimen is plasma, it should be clearly noted on label that the tube contains plasma.
  • All aliquots submitted must be accurately and clearly labeled.
  • Lab couriers will have dry ice available for frozen specimens.
